By checking this box, I declare that I have read and agree to abide by the Emily Aarons Holistic, LLC Terms & Conditions of Service, and I understand that this is a renewing subscription that will automatically charge on either a monthly or annual basis (depending on the type of subscription you chose). And that the charge will show on my credit card statement as Emily Aarons Holistic, LLC.

I understand that if I choose the monthly payment option, I am entering into a minimum 6-month term and cannot cancel my payments until after this initial 6-month commitment. 

In the event that I wish to cancel my subscription, I understand that I must give at least 5 business days notice by email prior to my next billing date or else a charge will be levied against my account. And I understand that if I dispute a payment directly with my financial institution for any reason, then I may incur a $30 admin fee per payment disputed.
